· Visits all uncovered villages conduct meetings with selected CHWS, ensure screening children & PLWs checked Consumption Zinc MNP and IFA,

· Strongly meeting conduct between respected NAs & CHWs ensuring referral of severe active case finding to treatment

· Supporting CHWs in Self Help Group, Mother Support Group & Hearth sessions, collecting monthly reports compiled in my report submitted to Nutrition specialties.

· Additional tasks complete the carpet screen in Jam Nawaz Ali taluka and make a micro plan for each village with the support of local contact persons’s complete tasks with 5 teams within 17 days.

· Educate clients or patients on healthy eating and nutrition. Evaluate patient's dietary habits and help them develop a plan for healthy meals. My job duties are to conduct patient interviews, provide dietary guidance, and report patient progress to the dietitian in charge, medically complicated patients to refer Medical Officer and NSC enrolled SAM patients Less < 11.5 MUAC ++ edema Grade +edema Grade Maximum stay time four Months Minimum two month to give them RUTF on weight, conduct session Health, Nutrition and IYCF, Routine medication Follow ups.

Responsibilities

· Community sensitization on the program objectives and method to stimulate & eventually promote the understanding and participation of the target population. Community leader and eventually extends to the entire population.

· Elicit community involvement in case finding, referral, encouraging attendance, and minimizing defaulters. Follow-up issues related to low attendance rate and defaulters and the underlying problem and propose appropriate solutions.

· Formation of hearth committees/VC/mother support group and close coordination on daily basis.

· Anthropometric measurement of children and PLWs at heart site/community level.

· Conduct health/hygiene/PD-Hearth Nutrition awareness sessions in the community/hearth site.

· Identification of hearth site through VC under the guidance of PD. Hearth supervisor.

· Jointly work with FSL, WASH, DRR team at field level in all activities and close coordination.

· Any other task assign by the supervisor to maintain quality program.

· Ensure ongoing community mobilization activities including satisfactory active case finding, referral & follow ups at hearth site & health facilities.
