Medical ward Unit-1, Dr. Ruth K.M Pfau, Civil Hospital

Being an intern doctor in a medicine ward is a challenging and rewarding experience that marks the transition from medical school to independent practice. It's a period of intense learning, where I gained practical experience under the supervision of senior doctors and nurses. Here's a glimpse into a typical day:

Morning:

  • Handover: I receive a report from the night staff about the patients' overnight status, highlighting any critical changes or concerns.
  • Rounds: Accompany senior doctors as they visit each patient, discussing their medical history, current condition, and treatment plan. I actively participated by presenting my own assessment and recommendations, which are then reviewed and refined by the seniors.
  • Patient Interactions: I independently examine assigned patients, taking detailed medical histories, performing physical examinations, and documenting my findings. This involves communication, critical thinking, and applying my medical knowledge to real-world scenarios.
  • Ordering Investigations: Based on my assessment and discussions with seniors, I may order necessary diagnostic tests like blood work, X-rays, or CT scans to confirm or rule out suspected diagnoses.

Afternoon:

  • Reviewing Results: Analyze the results of investigations and discuss their implications with the senior doctor. This often involves interpreting complex data and making informed decisions about next steps in patient care.
  • Implementing Treatment Plan: Collaborate with nurses and other healthcare professionals to administer medications, manage intravenous fluids, and provide other forms of treatment as prescribed by the senior doctor.
  • Patient Education: Explain diagnoses, treatment plans, and potential side effects to patients and their families in a clear and compassionate manner. This requires effective communication skills and empathy.
  • Documentation: Meticulously document all patient interactions, assessments, interventions, and progress notes in the electronic medical record. This ensures clear communication and continuity of care.

Evening:

  • Preparing for Next Day: Review patient charts, anticipate potential issues, and prepare for upcoming rounds and discussions.
  • Night Call: Depending on the schedule, I may participate in night call approximately 6 calls/month, providing care for new admissions and managing any emergencies that arise during off-hours.

Throughout the Day:

  • Learning Opportunities: Attended educational conferences, lectures, and workshops to broaden my medical knowledge and stay updated on the latest advancements.
  • Collaboration: Work effectively with nurses, pharmacists, social workers, and other healthcare professionals to provide comprehensive and coordinated patient care.
  • Self-reflection: Take time to reflect on my experiences, identify areas for improvement, and seek guidance from senior doctors and mentors.

Being a medical intern in a medicine ward is demanding, requiring long hours, physical stamina, and emotional resilience. However, it's an invaluable experience that equips ourselves with the necessary skills, knowledge, and confidence to become a competent and compassionate physician.

Just started my last intern year rotation, that is Psychiatry, one of my favorite. Its been only 9 days so far since the rotation started, I already have started to love this field more and more. In Emergency Dept of Psychiatry, we see multiple patients everyday with diagnosis of Schizophrenia, Bipolar Disorder, Personality Disorders, Substance intoxication. We admit those patients who match our admission criteria and look after them. History taking and counseling are so far, best parts in Psychiatry, that i enjoy very much. Looking forward to make most of this 3 months rotation.
